Cancellation Policy | No-Show Policy | Late for Scheduled Appointment Policy
At Arizona Sight Solutions, our goal is to provide exceptional, patient-centered care and treatment for your vision loss and overall health in a timely manner. In order to do so we have to implement an appointment cancellation policy. The policy enables us to better utilize available appointments for our patients in need of care and treatment.
Cancellation Policy & No-Show/Missed Appointment Policy
We understand that there are times when you must miss an appointment due to emergencies or obligations for work, school or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly "full" appointment book.
A "No-Show" is someone who misses an appointment without canceling it one business day in advance. No-shows inconvenience those individuals who need access to care and treatment in a timely manner.
True emergencies are an exception to this policy. Please note, however, that work and school-related demands are not considered emergencies.
How to Cancel Your Appointment
If it is necessary to cancel your scheduled appointment, we require that you call at least one business day in advance. Appointments are high in demand, and because we do not double book, your appointment time cannot be used by anyone else if you cancel on short notice or do not show up. Your early cancellation will give another person the possibility of access to timely health care and treatment.
To cancel an appointment, please use the Manage My Appointment button included in your Appointment Confirmation or Appointment Reminder emails. You may also call our office between 8:00 am - 5:30 pm at (480) 415-9547 or cancel via e-mail at least one business day in advance at [email protected].
Late for Scheduled Appointment Policy
We understand that delays can happen, however, we must try to keep the other patients and our practitioner on time. If you are running late, please notify the office.
If you are more than 15 minutes late for your appointment, you may have to reschedule and will be charged a late cancellation fee.
Late Cancellation and No-Show/Missed Appointment Fees
The following are charges for services in the office:
• Late Cancellation Fee – $25.00
• No-Show/Missed Appointment Fee – Full Amount (100%) of Payment for Service
Patients who habitually arrive late, fail to show up, or cancel on short notice will be dismissed from the practice.
We do not want missed appointment fees to be an impediment to your care and so we ask for your mutual respect. We block out our schedule for you when you make an appointment and ask that you exercise the same consideration when planning your calendar.
Habitual No-Shows, Short Notice Cancelations & Tardiness
Patients who habitually arrive late, fail to show up, or cancel on short notice will be dismissed from the practice.
